Long Term Condition Reviews

Our clinical pharmacists are trained to review a wide range of long-term conditions including (but not limited to) asthma, COPD, diabetes, hypertension, atrial fibrillation, heart failure, chronic kidney disease, and mental health conditions. They are particularly effective with patients who have polypharmacy needs (taking multiple medications) and can provide comprehensive reviews for patients with multiple co-existing conditions, ensuring an integrated approach to care.

Our reviews are specifically designed to satisfy all relevant QOF indicators for each condition. We ensure that appropriate tests are requested, results are recorded and interpreted, medication is optimised, and care plans are updated. Our clinical team stays current with the latest QOF frameworks and NICE guidelines, ensuring your practice meets quality standards and maximises QOF points. We provide detailed documentation that satisfies audit requirements and supports your QOF submissions.

Our service can be delivered either remotely or on-site, depending on your practice's preference and patient needs. Our remote review service utilises secure access to your clinical systems to review patient records, conduct telephone or video consultations with patients, and document findings. For practices preferring on-site support, our pharmacists can work from your premises, integrating directly with your team and conducting face-to-face patient consultations. Both options deliver the same high-quality clinical outcomes.

We work closely with your practice team to establish clear communication channels and workflows. Our pharmacists provide detailed documentation in your clinical system and can participate in relevant MDT meetings. We establish agreed protocols for escalation and referral back to GPs when necessary. Regular progress reports keep your team informed of outcomes, interventions, and any concerns identified.
